If you have been diagnosed with Hodgkins lymphoma, or treated with a bone marrow transplant, stem cell transplant, or CAR-T therapy you will have been told by your healthcare team that in the event you need blood products in the future, you require lifelong irradiated blood products. This information should have been added to your patient records, but as an extra precaution there is an irradiated blood products card that you can carry with you, you may have already been given this by your healthcare team. This might be particularly important if you are not attending your usual hospital.

If you need a replacement card, there are a couple of options:

• Open the link below, at the bottom you will find the irradiated blood card, you can print this, so you could write your details on the paper version - Or you could add text to complete the card online with your details.
• Take a photo of it
• Download an app such as: Pass2Uwallet, Pass4Wallet or NeatPass
• Open the app you have chosen
• Create New pass
• Add the image
• Add to apple wallet

• Add your photo to Photos – favourites – search “irradiated” in your photos to pull it up

Or

• Use the files app to save the PDF irradiated blood card document
