Paediatric Palliative Care at Nottingham Children’s Hospital is delivered by a small team, working alongside an in reach team from Rainbows Children’s Hospice. They are available during the working week.

The team supports babies, children and young people between 0-18 with a life limiting or life threatening diagnosis;  this frequently includes children undergoing active treatment who may need support with symptom management, team coordination, psychosocial support or  leadership for complex discharges. The team will develop an appropriate care plan and offer of services with the child and their family following assessment.

The ethos of the team is an emphasis on living with life limiting illness, and focuses on parallel planning and ensuring that children and young people have the opportunities of childhood, their wishes and choices heard, and their care delivered safely and effectively at home, hospital or hospice.

The team have particular expertise in advance care planning, symptom management, building an effective team around the family and end of life care.

Referrals can be made by families, ( via their bedside nurse), or by any member of the MDT. There is a nervecentre referral in development but currently please email or phone the team, or make contact during ward rounds and speciality MDTs which they attend regularly.

A CNS can support BCYP and their families in hospital from the point of diagnosis or recognition, throughout the child’s life, including providing end-of-life care. We can liaise between the hospital and Rainbows as well as planning with community teams for care at home, supporting patients, families and professionals. We can support with symptom management, advanced care planning, decision making, and choice of place for end-of-life care. The focus is on the BCYP quality of life and how we can support this throughout their life with holistic care for them and their family. Giving C&YP and their families choices and opportunities for their wishes to be heard and reflected in their care. BCYP does not need to be referred to Rainbows to have CNS support as inpatient. Referral to CNS is by phone or email to discuss support needed.

Hospital based family support worker is also available in UHL and QMC to BCYP and their families. 

BCYP under care of CYPICS can be referred to Rainbows by any professional that knows them well, including as much clinically relevant information as possible to ensure the criteria for referral is documented.
